Job application form — 40 free employment application templates for small business
A short, fair job application form helps a small business get more applicants and compare them consistently. This page explains what to include and the mistakes to avoid. Then it gives you 40 copy-paste blocks: eight application forms (short, full, cleaner, seasonal crew, stylist, driver add-on, part-time, rehire), wording for each section, safer alternatives to risky questions, hiring messages, and screening tools: phone screen, scorecard, interview and reference questions, and a new hire checklist.
General business information, not legal advice. Hiring laws vary by state and city.
A job application form collects the same basic information from every applicant: how to reach them, their availability, work history, references and a few job-related questions. That lets you compare candidates fairly and quickly, without relying on whoever has the best résumé.
For small service businesses, many good applicants don’t have a résumé at all, so a short, phone-friendly form gets more applications. The best forms are short, ask only job-related questions, and avoid questions that create legal risk. They also make it easy for you to reply to everyone.
What to include
Contact details and position applied for.
Availability grid and earliest start date.
Work authorization and minimum-age questions (asked the safe way).
Work history with permission to contact, plus references.
Job-specific questions: licenses, driving, physical tasks (“with or without reasonable accommodation”).
A certification statement and signature.
An equal opportunity statement, e.g., “We are an equal opportunity employer.”
Mistakes to avoid
Asking about age, citizenship, family, religion, health or salary history.
Including criminal history questions where “ban the box” laws apply.
Burying background check consent inside the application.
A long form that needs a laptop. Most hourly applicants apply from a phone.
Never replying. A two-line “not moving forward” text protects your reputation.
Using different questions for different candidates.
A worked example
A cleaning company owner was getting about 4 applications a month with a three-page PDF that required a résumé upload. She replaced it with a one-page phone form (the short application above), added pay and schedule to the job post, and promised a reply within 2 business days.
Applications rose to about 15 a month. She used the 10-minute phone screen and the scorecard for every candidate, did paid 3-hour working interviews for the top three, and hired two people who were still with the company six months later. The scorecards also made it easy to explain hiring decisions consistently.
Job application forms to copy
Paste into a doc, a Google Form or print it. Remove anything you don’t need.
Short one-page job application
Enough for most small service businesses.

Background check notice (separate page)
Federal law requires a standalone disclosure.
If you run background checks through a consumer reporting agency, the Fair Credit Reporting Act requires a separate, standalone written disclosure and the applicant’s written authorization. Don’t bury it in the application. Your screening company usually provides a compliant form. State and local laws may add rules about timing and criminal history.
Questions to avoid and what to ask instead
Some common questions create legal risk. These are safer swaps (general information, not legal advice).
Age and date of birth
Ask about the legal minimum only.
Avoid: “Date of birth?” “How old are you?”
Ask instead: “Are you 18 or older?” (or the minimum age the job legally requires).
Citizenship and national origin
Ask authorization only.
Avoid: “Are you a U.S. citizen?” “Where were you born?” “What’s your native language?”
Ask instead: “Are you legally authorized to work in the U.S.?” If a language is required for the job: “This role requires speaking with clients in Spanish. Are you able to do that?”
Family, pregnancy and childcare
Ask about the schedule, not the reason.
Avoid: “Do you have kids?” “Are you pregnant?” “Who watches your children?”
Ask instead: “This job is Mon–Fri 8:00–4:30 with some Saturdays. Can you work that schedule?”
Health and disability
Ask about essential tasks.
Avoid: “Do you have any medical conditions?” “Have you filed a workers’ comp claim?”
Ask instead: “Can you perform the essential tasks of this job, with or without reasonable accommodation?”
Salary history
Banned in many states and cities.
Avoid: “What do you earn now?” “What was your salary at your last job?”
Ask instead: “What pay are you looking for?” (Many states and cities ban salary history questions. Check yours.)
Criminal history on the application
“Ban the box” laws are common.
Many states and cities don’t allow criminal history questions on the application, or until after a conditional offer. If you need a background check for the role, explain it in your posting and follow the timing rules where you hire.

Common questions
Do small businesses need a job application form? It isn’t legally required, but it makes hiring faster and fairer. You get the same information from everyone and a signed statement that it’s true.
Can I ask for a Social Security number on the application? It’s best not to. Collect it after hiring for payroll (Form W-4) and the I-9 process, and store it securely.
How long should I keep applications? Federal rules generally require keeping applications and hiring records for at least 1 year (longer in some cases and states). Keep them secure.
Is a Google Form okay? Yes. Keep it short, mobile-friendly, and limit who can see responses.
Can I require a background check? Often yes, with a standalone disclosure and written authorization under the FCRA, and following state and local timing rules. Use a reputable screening company.
Is this legal advice? No. This page is general business information. Employment rules vary by state and city.
How to use this template
Pick the short form or the full form, add the job-specific add-on, and put it in a Google Form or a printable page. Post the job with pay and schedule, reply to everyone, and use the same phone screen and scorecard for each candidate. When you choose someone, send an offer letter, then set their hours on the work schedule.
